Plastic Surgery consists of two main fields:
Cosmetic Surgery and Reconstructive Surgery.

Cosmetic Surgery

Reconstructive Surgery

Cosmetic Surgery is related to the improvement of the physical appearance of people without any physical deformities. Usually, the aim of an Cosmetic surgery is to boost one’s self esteem by emending what is considered by the patient as a physical flaws. Most common treatments are augmentation mammoplasty or reduction mammoplasty (breast augmentation or reduction), rhinoplasty (septum), blepharoplasty (eyelids), liposuction (removal of the subcutaneous fat).

Cosmetic medicine, a branch of Medicine which has the same intent of Cosmetic Surgery, embraces slight interventions without the use of any surgery. It includes injection of botulinum toxin, electroporation, intense pulsed light and many others procedures.

Reconstructive Surgery refers to all those surgeries dedicated to people with physical deformities. Either for an acquired condition (due to a trauma or pathology) or an inborn one, it attempts to restore the function of the body and the aesthetic appearance.

Essentially we resort to reconstructive surgery on the basis of three main causes: traumas (burns, facial traumas, leak of substances, etc.), congenital diseases (cleft lip and cleft palate and others) or extirpative surgery (mastectomy, neoplasia, facial neoplasm, etc.)

Regenerative Surgery

Regenerative Medicine and Surgery, taken together, represent the most innovative application of science in medical field

The non-invasive approach, linked to the usage of patient's own tissue or cells, allows a shorter postoperative clinical course. In this way, physical and psychological stresses are reduced and healing processes are enhanced.

Regenerative Surgery is based on three key elements: synthetic biomaterials, stem cells from adipose tissue or other tissues, growth factors in platelets. Both stem cells and platelets are taken from the same patient, through an auto-transfusion.
In the case of interventions on bone tissue (as dysodontiasis, dentoalveolar wounds, fractures or leak of substances), it is used a concentrate of platelets obtained by collecting 18cc of blood. As a result of a particular centrifugation of the blood, we will obtain PRP (platelet-rich-plasma) which is rich in platelets and growth factors.
Regarding soft tissue regeneration, PRP can be used alone in facial and hands biorevitalization in combination with adipose tissue rich in stem mesenchymal cells. To do so, the patient undergoes an adipose tissue sampling through a liposuction aspiration, usually on the abdomen, thighs or flanks as patient prefers.
The tissue obtained undergoes a particular centrifugation which performs the purification of adipose tissue that will be added to the PRP previously obtained. Adipose tissue in addition to the PRP is injected through microinjections into the area in need, as skin dimpling, facial hemiatrophy, scar tissue, leak of substances, ulcers, or in minimally invasive aesthetic surgery in facial rejuvenation and breast augmentation.

In every quoted case, the aim is to enhance healing processes and to perform a tridimensional reconstruction of damaged tissue. Regenerative Medicine and Surgery are mostly performed in severe, hard to heal wounds and chronic ulcers therapy.

Techniques and methods:
Platelet gel and growth factors

Among various methods applied in tissues regeneration and restoration, platelet gel and growth factors have emerged as a new treatment procedure. Platelet gel allows the application of growth factors in terms of platelet-rich plasma (PRP) to enhance early stage wound healing (through bFGF, PDGF e IGF) and late-stage wound healing (through EGF, VEGF, TGF-b, IGF) in soft and bone tissue. It is s prepared from a small volume of blood (18 ml) harvested from patient before undergoing surgery, with approval of the Transfusional service. It can be used alone or in combination with adipose tissue graft.
It performs alone in extirpative oral and maxillofacial surgery, in bone tissue severe leaks that lead to dysodontiasis, in dental implants whereas there is an absence of bone tissue for the positioning of the implants, in edentulism, in maxillary sinus floor augmentation and in dentoalveolar wounds.
It performs together with fat grafts centrifugation (containing mesenchymal stem cells) in decubitus ulcers treatments, chronic non-healing wounds, trophic and vascular lower-extremity ulcers, post-traumatic loss of substances, hemifacial atrophy, Romberg syndrome, scar tissues, burns sequelae, breast reconstruction and minimally-invasive plastic surgery.

Stem cells
Stem cells are undifferentiated or non-specialized cells in the organism. They can reproduce themselves many times over and differentiate in every progeny cells of organs various type. They can be totipotent, pluripotent or unipotent. Adipose tissue is rich in mesenchymal stem cells that can stimulate damaged soft tissues regeneration.
Cell separators

Mesenchymal stem cells graft and separation can be performed troughs cell separators.

Lipoaspiration is performed and the fat is divided in two portions. The first portion is processed in the cell separator to extract mesenchymal stem cells. The second one is combined to the mesenchymal stem cells so as to be grafted through a micro cannula and multiple tiny adits the area being treated.

Biotechnologies and biomaterials
Thanks to tissue engineering, biotechnologies aim at in vitro growth and reconstruction of tissues and organs on three-dimensional biocompatible polymers matrixes. Main options are tissue banking (homologous or harvested from cadaver), tissue-engineered skin (autologous or homologous expanded in vitro) and synthetic biomaterials. These are mainly hyaluronic acid-based and collagen and are used in ulcer advanced healing process.
Re-cell

Re-Cell or ReCell Spray-on Skin is a novel autologous cell-transplant device for discolored skin patches treatment as vitiligo, burn scars, and other type of leukoderma in critical body zones (face, hands, genitals).
A thin split-thickness cutaneous biopsy is harvested (1-2 cm2) in easy to hide body area. The device produces a suspension that is sprayed onto the areas of scarring, previously surgically treated.
